Seeking Advice and Sharing Experiences with Urostomy

This topic is about managing a urostomy when dealing with incisional hernias and finding the right appliances that fit well. The person who started the discussion is facing two main challenges: dealing with hernias that developed after surgery and having trouble finding equipment that works well with both the hernias and the stoma. Someone suggested that the stoma might be in a difficult position and could be moved, but before considering surgery, the person is looking for practical advice from others who have a urostomy.

Here are some helpful suggestions:

1. It might be beneficial to consult with an ostomy nurse for a personalized assessment of the stoma placement, the impact of the hernia, and how well the appliances fit. This can be done in person or through Zoom meetings with support-group nurses.

2. There are several appliance and accessory options that might improve the fit around hernias:
- A convex wafer or barrier can help create a tighter seal against uneven surfaces.
- Using an ostomy belt with a convex wafer can help keep the flange pressed evenly against the abdomen.
- Moldable rings, like the Eakin Cohesive Seal, can fill deeper or irregular areas around the stoma before applying the wafer.
- Ostomy pastes, such as Hollister Adapt Paste, can fill gaps and help prevent leaks.

3. Instead of relocating the stoma, consider surgical repair of the hernias. Hernias can worsen or change over time, and repairing them might resolve leakage issues without needing to move the stoma.

4. Peer support is available. Experienced urostomy users and support-group leaders can offer one-on-one help and can also relay questions to ostomy nurses during virtual meetings.
