Playing Racquetball After Radical Cystectomy - Stoma Protection Advice Needed

The topic is about returning to playing racquetball after undergoing a radical cystectomy, with a focus on how to protect your stoma during the game. Here are some helpful tips and insights shared by others who have been in a similar situation:

1. Basic protection: Many people find that wearing a snug ostomy belt or an abdominal wrap is usually sufficient to keep the pouch flat and secure while playing.

2. Extra impact guard: If you're concerned about direct hits, consider using a "stoma dome," which is a rigid plastic cup that can be attached over the pouch outlet beneath the belt or wrap.

3. Combined approach for vigorous play:
- Wear a 4-inch hernia belt tightly along with a stoma dome for maximum protection.
- Secure the dome to the belt or pouch with tape to prevent it from shifting during play.

4. Sweat management and flange security: To prevent the flange from lifting due to perspiration, one player suggests taping it with extra Opsite Tape (3M) before a match.

5. Post-surgery core changes: Be prepared for reduced core strength after surgery. You might need to adjust your playing style by incorporating more lobs, drops, and boasts, and start back slowly.

6. Hernia caution:
- Parastomal hernias are common, and one person experienced a large bulge within a year despite quick healing.
- Fast, twisting sports like racquetball may increase the risk of hernias, so it might be a good idea to discuss this with your surgeon before resuming the sport.
- If you decide to play, always use a hernia belt and dome guard, but keep in mind that these devices only provide external support—the initial damage occurs inside the abdominal wall.
