Struggling with Stoma Leakages and Skin Irritation - Need Advice

This topic is about managing severe leakage and skin irritation around an inverted urostomy stoma. The person experiencing this issue is dealing with urine pooling around the stoma, causing painful skin irritation. Despite trying various products, the problem persists, leading to emotional distress. Here are some helpful suggestions and insights:

1. Seek medical support:
- Consult a urologist or general practitioner for antibiotics if the skin seems infected.
- If you're not confident in your current consultant, consider finding a specialist with experience in urostomy care.

2. Improve skin protection and healing:
- Ensure the area is completely dry before applying the pouch. Use stoma powder and pat it with a barrier wipe until it disappears. Repeat this process 2-3 times.
- If there's a fungal rash, use an anti-fungal treatment first, but avoid applying wafers over oily preparations.

3. Switch to a convex system:
- A convex wafer or pouch can help push the stoma forward, preventing leaks. Consider asking Hollister for a convex Moderma Flex or Flextend-convex wafer.
- Other convex products are available from Coloplast and ConvaTec, and you can request free samples.

4. Fill gaps around the stoma:
- Use a strip of paste, like Coloplast Strip Paste or Hollister Adapt paste, to fill any voids around the wafer.
- Eakin Cohesive Seals are also effective as they conform to uneven skin and support the wafer.

5. Full wafer fitting routine:
- Remove the appliance and shower freely.
- Dry thoroughly and trim any hairs.
- Dust with stoma powder, pat with a barrier wipe, and repeat up to three layers.
- Apply a barrier film or wipes.
- Apply the prepared convex wafer with paste or seal, pressing gently for several minutes. Heat can help set the bond.
- Avoid bending or sitting for about 30 minutes to let the bond set.
- If the wafer edges lift, secure them with tape.

6. Additional leak-prevention tips:
- Change appliances every 5-7 days or at the first sign of itching.
- Tape the top and sides of the wafer before showering if not changing it.
- Experiment with different combinations, as each stoma is unique.

- Reach out to Coloplast, ConvaTec, and Salts Healthcare for helplines, samples, and guidance.

8. Encouragement:
