Irritated and Painful Skin Around Wafer

This topic is about dealing with painful, itchy skin reactions caused by the adhesive wafer of a 2-piece Hollister ostomy system. If you're experiencing discomfort and irritation in the skin under and around the adhesive, here are some practical tips and insights that might help:

- You might be allergic or sensitive to the adhesive used in the Hollister product.
- Try requesting free sample wafers or barriers from other Hollister lines, as well as from brands like Coloplast and ConvaTec, to see if a different adhesive works better for you.
- Some people have found relief only after switching to a different brand.

- Make sure to remove all traces of the old adhesive.
- Use an adhesive-remover wipe or spray first, then clean the skin thoroughly and let it dry before applying a new wafer.
- Follow up with a skin-barrier wipe or spray, such as Cavilon, before attaching the next appliance.

- Consider using protective products that others have found helpful:
- Coloplast Brava Barrier Cream: Apply a thin layer from the stoma outward, past where the wafer edge will sit. Make sure it is well-rubbed-in so the skin feels slightly tacky before applying the wafer.
- Coloplast Brava Ring: Warm it in your hands first, apply to dry skin, then apply the warmed wafer or pouch.
- Lightly dust ostomy or fungal powder on raw or weepy spots, and brush off any excess.
- For severe redness, short-term use of cortisone cream and an antifungal cream can help. Apply, wait 15-20 minutes, then wash off with a mild soap like Cetaphil before setting up your pouching system.

- Here is a step-by-step change routine suggested by several members:
1. Remove the appliance and adhesive with a remover wipe.
2. Optionally, take a shower for a thorough, soap-free rinse.
3. Address any active irritation with cortisone, antifungal cream, or powder as mentioned above.
4. Rinse or wipe with warm water only, and pat completely dry.
5. Apply Brava Barrier Cream at every change, even when the skin looks healthy.
6. Apply a skin-barrier wipe or spray, like Cavilon, and allow it to dry.
7. Warm the Brava Ring in your hands, place it around the stoma, warm the wafer similarly, and apply.
8. Ensure the opening fits exactly with no exposed skin. Hollister’s printable oval templates can help with this.

- Mechanical issues might also contribute to soreness:
- A tight ostomy belt can cause pressure and friction. One person found relief by removing the belt, using a stoma collar, and changing the collar daily.

- If these steps do not help, it’s important to consult your stoma or ostomy nurse promptly for a hands-on assessment and product recommendations.
