Has Anyone Else Had a Flange Separate from the Bandage Wafer?

This topic is about an 81-year-old who recently had ileostomy surgery and is experiencing issues with their ostomy appliance. They are using a Hollister convex wafer in a two-piece system with an Eakin barrier ring. During a nurse visit, a small leak was found because the plastic flange started to separate from the wafer's adhesive part. The nurse suggested that pressure from underwear or elastic waistbands might be causing this issue, which has made the person worried about their clothing affecting the appliance's security.

Here are some helpful tips and insights:

1. Proper skin preparation is essential:
- Use adhesive remover generously, then clean the area with a wet cloth and make sure it is completely dry.
- Apply Stoma Powder around the skin near the stoma, then wipe it all off using the "crusting" technique before applying a liquid skin barrier. This helps create a dry, flat surface.
- The Eakin barrier ring is highly recommended to help reinforce the seal.

2. If the flange continues to separate from the wafer, consider the possibility of a product defect rather than blaming clothing pressure. Try opening a new box of Hollister flanges or using a different lot number.
