Ideal Pouch Hole Size for an Ileostomy with a Convex Bag

This topic is about finding the right hole size for a convex ileostomy pouch. After undergoing surgeries that resulted in a new ileostomy, the person is trying to figure out how snug the wafer opening should be to prevent skin irritation while avoiding pressure or abrasion to the stoma. Here are some helpful tips and insights shared by others:

1. Gap around the stoma:
- Many suggest leaving a 1 mm space (about ¹⁄₃₂") between the wafer and the stoma. This is tight enough to keep waste off the skin but loose enough not to scrape the stoma.
- A wound-care nurse recommends allowing about 1⁄8" (approximately 3 mm) of "wiggle room" to accommodate the stoma’s natural expansion and contraction.
- Be cautious: a hole that is too small can injure the stoma, while one that is too large can increase the risk of skin irritation.

2. Sealants and fillers:
- Use a barrier ring, stoma paste, or strip to ensure a snug seal where the convex wafer cannot sit flat.
- Adapt Paste (Hollister) is mentioned as a helpful product; applying a bead around the base before putting on the pouch can help fill the channel and protect the skin.

3. Accessory products:
- Consider trying the Conseal Stoma Collar, which is made of soft, flexible rubber. It protrudes up the sides of the stoma, shielding tissue and directing output directly into the pouch. It might be worth requesting samples.

4. Skin protection:
- Always use a skin-barrier wipe or film under the wafer. This provides extra protection in case of minor leakage.

5. Expect change and experiment:
- A new stoma usually changes size for about six weeks, so measure it before each change and adjust the cut size as needed.
- Try different wafers, pastes, and collars. Manufacturers often provide free samples for you to test.
