Struggling with Ileoconduit Wafer Seal Leaks

The topic is about someone who has recently started using an ileoconduit and is facing challenges with wafer-seal leaks. This can be frustrating as it leads to frequent changes and rapid use of supplies. Here are some practical tips and advice that might help:

1. Fit of the Wafer:
- Ensure the opening is cut just large enough for the stoma, with about 1/16 inch clearance. A wider cut can lead to leaks and skin irritation.

2. Skin Preparation:
- Use non-perfumed soap, like Ivory or Pure and Natural, to clean the area and avoid any residue.
- Make sure the skin is completely dry before applying the wafer. Some people use a hair-dryer to ensure dryness.
- Consider using a protective barrier:
- 3M Cavilon No Sting Skin Prep spray, also available as Nexcare No Sting Liquid Bandage.
- Smith & Nephew Skin-Prep wipes, which work well with ConvaTec wafers but are not recommended for use with Hollister wafers.

3. Seal-Building Products:
- Apply a thin bead of Stomahesive Paste around the cut hole to act as a gasket.
- Use Eakin Cohesive Seals by stretching them to fit snugly around the stoma before applying the wafer. Free samples are available.
- Friars Balsam can be dabbed onto the wafer area with a sea sponge after showering. Let it become tacky before applying the wafer to improve adhesion.

4. Managing High Output:
- Some people manage high output by using higher doses of codeine, but this should only be done under medical supervision.

5. Product Brands:
- Many users find success with the Hollister one-piece system for better leak control.
- The ConvaTec Combihesive Natura flanges and pouches, combined with Stomahesive paste and Karaya seal, are also recommended.
- Duoderm Extra Thin dressing can be used under the flange to protect sore skin.

6. Professional Help:
- Consider consulting a certified Wound, Ostomy, and Continence (WOC) nurse for personalized advice and support.

7. General Tips:
- Order free trial samples from manufacturers’ websites to find the most reliable system for you.
- Leaks often decrease as the stoma and abdominal contours settle over the first year, so patience and regular reassessment are key.
