Post-Ileostomy Reversal - Severe Diarrhea, Pain, and Vomiting Issues

This topic is about someone who recently had an ileostomy reversal after having parts of their colon and small intestine removed due to a blood clot and septic shock. Since the reversal, they have been dealing with some challenging symptoms, including severe diarrhea, abdominal pain, and vomiting. Here are some helpful insights and advice for managing these issues:

1. Clarify the type of reversal procedure you had, such as end-to-end, side-to-side, or J-pouch. It might also be useful to request a stool sample to rule out any infections like C. difficile.

2. Review your current medications. For instance, Valium (diazepam) can relax muscles and might worsen fecal incontinence. It's important to understand why it was prescribed and if there might be a safer alternative.

3. The combination of pain followed by vomiting could indicate a temporary or partial bowel obstruction. Consider these diagnostic steps:
- Have a detailed review of your CT scan to check if the loops of your bowel are truly dilated or thick-walled.
- An MRI enterography (MRE) might be more effective than a CT scan in identifying small-bowel obstructions.
- A colonoscopy could be helpful since it can look far into the remaining small bowel and might identify strictures, kinks, or active disease.

4. It might be beneficial to seek a second opinion from a colorectal surgeon or gastroenterologist at a different hospital or clinic, as the accuracy of imaging can depend on the radiologist's expertise.

5. If your primary-care physician doesn't specialize in colorectal issues, ask for a referral to a tertiary-care center where you can get more specialized care.

6. For practical coping with nighttime accidents, consider using mattress and bedding protective covers until the underlying issue is resolved.

7. Remember to maintain hope. Many people have faced similar challenges and have seen improvements after getting evaluated by specialists and receiving the right treatment.
