Concerns about Constant Need to Go with Sigmoid Colostomy

This topic is about managing the challenges that come with having a sigmoid colostomy, especially when dealing with constant urgency, odor, and mucus. A sigmoid colostomy is a surgical procedure where part of the colon is diverted to an artificial opening in the abdominal wall, but in this case, the rectum and sphincter are left intact. The person in question is concerned about ongoing issues with bowel movements, pressure, and odor, and is seeking advice from others who have experienced similar situations.

Here are some helpful insights and advice shared by others:

1. Consider getting a second opinion from another colorectal specialist. They might confirm the current plan or suggest alternatives.

2. Experiences with a retained rectum can vary:
- Some people have minimal pressure and only a few mucous discharges daily, with little to no odor.
- Others experience uncontrollable, fishy-smelling mucus that can sometimes be blood-tinged and leak like thin oil.

3. For managing odor and leakage:
- Use ultra-thin disposable diapers, incontinence pads, or folded facial tissues to absorb mucus.
- Add a few drops of peppermint, spearmint, or eucalyptus oil, or use body spray on pads or diapers to help mask odors.
- A quality paper towel rolled and placed between the buttocks can help trap discharge.

4. Rectal irrigation might help reduce foul-smelling mucus:
- One person adapted a nozzle from an anal irrigation system to a shower hose for daily flushing, which eventually reduced the foul mucus to clear, non-smelly mucus.
- Irrigating twice daily or in the morning might help lessen pressure and odor.

5. Testing different anal continence products:
- Anal plugs can catch stool but may allow mucus to leak and can be uncomfortable when sitting.
- Women’s tampons might absorb mucus better than plugs but can slide out once saturated.

6. Living with ongoing clear mucus:
- Using incontinence pads with tissue inserts can allow for full-time work and normal activities.
- Surgery to remove the rectum can be considered in the future if these conservative measures are not sufficient.
