High, Low, or Does It Really Matter?

This topic is about choosing the best position for a stoma, especially considering clothing, potential leaks, and lifestyle needs. A woman preparing for a colostomy is trying to decide where her stoma should be placed. She wants to ensure it fits well with her clothing choices and lifestyle, while also minimizing any issues like leaks or hernias. Here are some helpful insights and advice:

1. Consult with an ostomy or stoma nurse to mark the site. They will check your body in different positions—standing, sitting, and lying down—to avoid natural creases and scars, which can help prevent future issues with wafers leaking or lifting.

2. The ideal stoma placement is often at the midpoint of a triangle drawn from the navel to the hip bone and down to the pubic bone. This position allows the bowel to pass through the abdominal muscle, reducing movement and the risk of hernias.

3. Placing the stoma too low can cause problems:
- Belts or waistbands might rub directly over the pouch, increasing wear issues.
- There could be more leaks with looser stool, as warned by surgeons.

4. Placing the stoma too high can also cause issues:
- It might interfere with tops, bras, or seat belts and be harder to conceal under clothing.

5. Clothing tips:
- Mid-rise or regular-rise jeans usually cover a stoma placed near the triangle center without compressing the pouch.
- One-piece swimsuits can easily hide the appliance.
- Low-rise fashions might expose the pouch if the stoma is set too low.

6. For swimming, consider using "mini pouches" (infant-sized ostomy bags) without filters to reduce bulk under swimwear. Many people find these work well.

7. It's important to have a say in stoma placement, as it can provide a sense of control before surgery. Don't feel trivial for caring about how your clothes fit—comfort and confidence are important in everyday life.
