Is Emptying My Iliostomy Pouch Frequently Normal?

This topic is about someone who has an ileostomy and is dealing with short bowel syndrome. They find themselves needing to empty their ileostomy pouch quite frequently, sometimes a couple of times an hour, and are concerned about whether this is normal. They are also managing their condition with total parenteral nutrition (TPN) and have a history of significant medical challenges. Here are some helpful insights and advice for managing this situation:

1. Consider using a longer or higher-capacity pouch. Even if you have a smaller frame, you can tuck or roll a larger bag inside your underwear. This allows the pouch to fill more before it strains the seal, which can help reduce how often you need to empty it.

2. Try using pouch-thickening products, like gelling granules or sachets, which you can place inside the bag. These can help firm up liquid output, making it easier to manage and less likely to cause leaks.

3. Ensure that the flange fits well. A secure seal allows the pouch to fill to a higher level without compromising the adhesive, which can help prevent leaks.

4. Over time, the remaining small intestine may adapt, which can slow down output and improve absorption. This natural adaptation can help reduce the frequency of emptying.

5. Talk to your doctor about anti-motility medications, such as codeine. Some people with short bowel syndrome use these medications to reduce the number of times they need to empty their pouch from around 30 times a day to about 6–10 times.

6. Frequent emptying, such as every time you urinate, is common among people with a small frame or high-output ileostomies. It is often a personal comfort choice and is considered "normal" for many individuals in similar situations.
