Still Passing Feces Out Anus Occasionally

This topic is about a person who, six months after having surgery to create two stomas (a loop ileostomy and a mucus fistula), is experiencing occasional hard, white anal discharge. This discharge causes a strong urge to go to the bathroom and requires several trips before it is expelled. The surgeon believes this is due to residual fecal matter from the time of surgery being expelled through a fistula to the anus. Further surgery is not planned due to high operative risk.

Here are some helpful insights and advice shared by others:

1. Many people confirm that the material being passed is likely a "mucus plug." This is dried or compacted mucus mixed with shed rectal cells and a small amount of stool left in the unused rectum. The color can vary from white to yellow, green, or even resemble "gold nuggets."

2. It is normal to pass mucus or small amounts of stool through the rectum when the rectum is left in place after a loop ileostomy or colostomy. The only way to completely stop this output is to remove the rectum entirely.

3. Dehydration can thicken rectal mucus, making it harder to pass. Drinking at least 8–10 glasses of water daily can help prevent the formation of hard plugs.

4. Intermittent rectal sensations and a lack of control when these plugs move are common. While uncomfortable, this usually does not indicate a problem.

5. People with loop configurations typically have one stoma for output and still need to expel mucus through the rectum. In less common cases, like this one, there is a second "mucus fistula" stoma in addition to the rectal expulsion.

6. If the plugs become painful or difficult to pass, it is advisable to consult the surgeon or a stoma nurse for guidance. Some individuals have required hospital removal under anesthesia when a plug was stuck.

7. For ongoing support, it is recommended to have regular access to stoma nurses. Some have found help through services like a Coloplast Customer Support Officer, who can arrange remote nursing assistance.
