Urostomy Pouch Failures - Seeking Solutions for Seepage and Adhesion Issues

This topic is about dealing with a challenging situation where a urostomy stoma is placed very close to an abdominal incision that is still seeping. The moisture from the incision is causing the adhesive on the urostomy pouch to fail, leading to leaks. Here are some suggestions and insights shared by others who have faced similar issues:

1. Consider creating or modifying your own baseplate. Some people have had success by making custom baseplates and using "Probond" prosthetic adhesive, which bonds well with standard pouches. There are also samples from Dansac with stronger glue that hold well, though they can be a bit tricky to remove.

2. Try cutting the wafer opening off-center. By making the stoma hole closer to one edge, you can rotate the wafer so that its adhesive part stays away from the leaking incision.

3. Focus on re-dressing the incision instead of the pouch. Keep the wound dry by using absorbent dressings or gauze, changing them several times a day. Make sure the incision is fully covered to catch any seepage before it reaches the wafer.

4. Use stoma powder generously around the damp area until the weeping stops. Some people have found that it takes about ten days after removing a drain for the moisture to subside.

5. Look into high-absorbency or hemostatic dressings. These are similar to products used for shaving nicks and can help stop or absorb fluid at the incision site.
