Tips for Changing a Closed Pouch with Limited Hand Function

This topic is about finding ways to change a closed two-piece ostomy pouch when you have limited hand function, specifically for someone who is quadriplegic. The person is looking for techniques or tools to make cleaning around the stoma easier, as they face challenges with hand dexterity. Here are some helpful suggestions:

1. Lightly moisten cleaning materials:
- Fold a tissue and dampen it with water. Rehydrating dried stool can make it easier to lift.
- Use moistened, pointed cotton swabs for detailed cleaning, focusing on making the outer flange spotless.

2. Create a disposable plastic "bib":
- Cut along one long side and the bottom of a disposal bag to make a large plastic sheet.
- Tuck one end into the waistband and seal the edge to the skin with tape, draping the lower end over a sink or container.
- While seated, pour water over the stoma so that the effluent and rinse water run down the plastic into the catch area.
- Remove the tape and discard or rinse the plastic sheet afterward. Any wide piece of plastic can be used as a substitute.

3. Consider a drainable system used solely for rinsing:
- Rotate a two-piece drainable pouch 180° to fill with water and flush the stoma before removing the appliance.
- If toilet access is not possible, funnel the output into a larger container that a caregiver can empty.

4. Use a small rinse cup:
- A shallow, clear plastic food cup can be filled with water and gently swished around the stoma to loosen stool. Then, wipe away any remaining residue with soft tissue.

5. Stage and protect the work area:
- Gather all supplies beforehand.
- Tuck an open garbage bag into the waistband or under the body so any spillage falls directly into it.
- Keep a large bowl of warm water on a bedside table to dip and wring paper towels for repeated wipes without needing a sink.
