Sudden Increase in Stoma Size and Leakage Issues

This topic is about a new ostomate who is experiencing some concerning changes with their stoma. The stoma has suddenly become larger, is moving in and out, and is leaking a watery, bloody fluid. This situation is understandably causing a lot of anxiety. Here are some helpful insights and advice to consider:

1. Identify the type of stoma you have, whether it's an ileostomy or a colostomy, as this can affect what is considered normal in terms of size and behavior.

2. Determine the source of the red fluid:
- If the bleeding seems to be coming from inside the stoma, it could be true intestinal bleeding, and you should contact your surgeon or seek emergency medical help immediately.
- If the bleeding is only on the outside of the stoma or around the wafer, it might be surface bleeding caused by friction or wiping, as stomas can bleed easily.

3. Consider whether food could be affecting the color of the output. Natural or artificial dyes, like those from beets, can make the output appear red.

4. Check the fit of your appliance:
- A wafer or barrier hole that is too small can constrict the stoma, causing it to swell and bleed on the surface.
- Appliances that don't fit properly can lead to irritation and leaks. Finding the right product and size can make a big difference.

5. Understand that changes in stoma size throughout the day are normal:
- Bowel movements can cause the stoma to protrude more when at rest and retract when active or about to expel output.
- In the first few months post-surgery, the stoma is still "settling," so its size and shape can change.

6. Consider the possibility of a prolapse if the stoma appears significantly longer than usual. Consult an ostomy nurse for an assessment.

7. Keep an eye on other symptoms like pain, diarrhea, more frequent emptying, sensations of blockage, or continued watery/bloody output, as these may indicate an infection or a stomach/intestinal issue. Seek medical evaluation if these occur.

8. When cleaning the stoma, use only water or approved ostomy wipes. Harsh cloths or chemicals can cause bleeding.

9. If you're unsure about anything, schedule a visit to your ostomy clinic or nurse. Professional measurement and appliance refitting can quickly resolve many early-stage problems.
