Help Needed: Correct Order for Applying Stoma Products

This topic is about finding the best way to layer various ostomy products to ensure a secure and comfortable fit, especially for someone with sensitive skin and a stoma that sits deep in the abdomen. The person in question has been using a specific routine but is experiencing some issues like minor leaks and early wafer lift-off. Here are some helpful tips and advice shared by others:

1. Suggested Layering Order:
- Start with the Brava Protective Sheet directly on the skin.
- Place a barrier ring (like the Adapt CeraRing or Brava/HyperSeal) on top of the protective sheet.
- Apply the SenSura Mio convex wafer/base plate.
- Attach the pouch.
- Use optional flange or extender strips if needed.

2. Ensure the stoma protrudes above the final surface after applying the sheet, ring, and wafer to prevent leaks.

3. Cut the wafer precisely to fit the stoma's size and shape, and re-measure periodically as the stoma may change over time.

4. Change the bag every 3-4 days to protect the skin and prevent adhesive breakdown, even though some people manage to wear it for 5-7 days.

5. Simplify the routine to test the necessity of each layer:
- Clean the skin with warm water and dry it completely.
- Use a single powder-spray "crusting" method if the skin is broken; otherwise, skip this step.
- Apply a warmed ceramide-infused barrier ring directly against the stoma.
- Press the wafer firmly for 2-3 minutes; using a handheld "pressure ring" tool can help ensure even application.

6. If output is heavy during changes, try eating 4 marshmallows 30 minutes before changing or schedule the change first thing in the morning after overnight fasting.

7. Warm all products (underarm, bra, or pocket) before application to improve adhesion.

8. If the honey-based HyperSeal promotes yeast, avoid using it directly on the skin; place it on top of the protective sheet or use another ring.

9. Contact ostomy product manufacturers’ helplines for guidance on application steps and request written or video instructions.

10. If issues persist, consult with a stoma nurse to observe a full change, provide written step-by-step instructions, and reassess the product load.

11. Consider other products that might help, such as:
- Hollister CeraPlus (ceramide) wafers.
- Coloplast Y-shaped and C-shaped elastic barrier extenders.
- Deep-convex wafers with only C-straps and an ostomy belt for a minimalistic yet secure fit.
