Dealing with Stoma Prolapse

This topic is about managing a recurrent ileostomy prolapse and dealing with leakage issues related to the use of a hernia belt. The person involved had emergency surgery that resulted in an ileostomy, and despite surgical revision, the stoma has prolapsed again. They are using a one-piece pouch and a hernia belt with a flap designed to prevent the stoma from protruding, but this seems to be causing output issues and leaks. Here are some helpful suggestions and insights:

1. Verify belt fit and positioning:
- Ensure the belt is worn correctly by checking its size, orientation, and tightness. It might be helpful to have an ostomy nurse assess it.
- Consider getting a second opinion from a surgeon or an ostomy nurse to understand why the prolapse keeps happening and whether the belt is beneficial or not.

2. Consider specialized prolapse-control belts:
- Nu-Hope Corporation offers ostomy support belts with an adjustable prolapse strap that uses Velcro. This allows you to adjust the pressure to flatten the stoma without blocking output. Many users find these belts helpful for both special occasions and everyday use. Contact Nu-Hope for guidance on sizing.

3. Belt/strap should not obstruct output:
- Position the strap to stabilize the stoma rather than compressing it. If the stoma points toward the midline, you might need to angle or offset the strap to prevent blockage, especially if there's a mild parastomal hernia.

4. Re-evaluate the need for further surgery:
- If the prolapse continues to be an issue despite using the belt correctly, discuss surgical options with a different colorectal surgeon. Consider the effectiveness of revision surgery, potential risks, and other alternatives.

5. General tips:
- To prevent pancaking, try lubricating the inside of the pouch or switching to a different one-piece system with a better internal coating.
- Use barrier rings or convexity that match your current abdominal shape. Frequent leaks might indicate the need to refit the appliance after changes due to prolapse.
