Need advice for flange not sticking to old stoma site

This topic is about finding solutions for a flange that won't stick properly over a dent left by a previous stoma site. The writer has a permanent ileostomy and is experiencing issues with the flange lifting, causing skin irritation. Here are some suggestions to help with this problem:

1. Consider using a smaller flange or appliance:
- A compact two-piece Convatec flange might avoid the puckered area.
- Hollister 3796 “Pouchkins” from the children’s line could be suitable for shorter torsos and small stomas.

2. Stick with Coloplast but try different models:
- The Coloplast Sensura shallow convex base plate, product 15537, is known for its flexibility and strong adhesion on uneven skin.
- The Coloplast Sensura is designed to move with the skin, with reports of no leaks when changed every two days.
- Keep an eye out for the upcoming Coloplast “Mio” pouch, which features a stretch-fabric flange that can lift and re-stick easily.

3. Fill the dent before applying the flange:
- Use ostomy paste, moldable “sticks,” or Play-Doh-like rings to fill the indented scar, then place the wafer on top.
- Stomahesive strips, which are double-sided and moldable, can be pressed into the void to create a flat surface.

4. Try additional products to manage output or improve the seal:
- Eakin or Vernagel gel sachets can help thicken watery ileostomy output.
- Pelican-Omnigon pouches are another brand option to consider.

5. Use application techniques for stronger adhesion:
- Clean the area thoroughly; some people find that washing with toothpaste removes residue and soothes the skin.
- Warm the wafer with a hair-dryer just before placement to activate the adhesive.

6. Seek vendor support:
- Contact Convatec’s customer service for advice on flange sizing and to request samples.
