Ostomy Pouch Issues Due to Tumor-Related Weight Gain

This topic is about finding ways to keep ostomy pouches secure when dealing with rapid abdominal enlargement due to a pituitary tumor. The tumor causes an increase in prolactin hormone, leading to significant swelling, which makes it challenging to keep ileostomy pouches in place. Here are some helpful insights and advice:

1. The tumor in the pituitary gland is causing an excess of prolactin, and surgery is not an option. The abdomen has expanded significantly, resembling a five-month pregnancy, which may be causing the wafer to peel or lift.

2. The current system being used is a ConvaTec one-piece pouch with a flexible wafer. Eakin Cohesive Seals and an ostomy belt have also been tried, but without success.

3. Daily visits from a home-care nurse are part of the routine, and there are plans to consult with a hospital ostomy nurse for further evaluation.

4. It is recommended to consult with an ostomy (WOC) nurse who is familiar with a wide range of products. A properly fitted system should ideally last several days.

5. Re-evaluate the application technique and the use of accessories. For instance, using too much adhesive powder can interfere with the pouch's ability to stick properly.

6. Consider whether factors like stoma placement, perspiration, or body contours are causing the wafer to lift. If so, look into products specifically designed for uneven or convex surfaces.
