Pressure sore ulcer next to stoma - How to heal and manage it?

1. Consult with a stoma or wound-care nurse as soon as possible. Regular professional treatment is essential for healing stubborn peristomal ulcers. If necessary, a surgeon might also need to be involved.

2. Consider the possibility of pyoderma gangrenosum, which is an extra-intestinal manifestation of ulcerative colitis, as a potential diagnosis.

3. Use protective or absorbent barriers to help manage the sore:
- An Eakin Cohesive Seal can be placed around the stoma to cushion the sore and block any output.
- DuoDerm hydrocolloid can act as a "second skin" over medicated dressings, allowing the wafer to be applied on top.

4. To soothe irritated skin, try dabbing Milk of Magnesia on the area. Let it air-dry or use a cool hair-dryer. The change in pH might help speed up healing.

5. Consider making adjustments to your appliance:
- If output tends to pool around the coupling of a 2-piece system, temporarily switch to a 1-piece pouch to minimize seepage onto the sore.
- Continue using an ostomy belt to keep the appliance snug against the skin and reduce leakage.

6. For those with diabetes, it's important to maintain strict hygiene, keep an eye out for signs of infection, and consider systemic antibiotics if drainage or redness increases.

7. Keep stoma powder handy for light weeping, but avoid heavy "caking" as it might prevent a snug seal.
