Flange/Barrier Cleaning Tips

This topic is about managing a colostomy, specifically focusing on keeping the flange or barrier clean and preventing pancaking, which is when stool sticks to the top of the pouch instead of sliding down. This can lead to soiling, odor, and leaks. Here are some practical tips and insights to help manage these issues:

1. Rinse-through method:
- Keep a squeeze or sports bottle filled with tap water in your bathroom. When you empty your pouch, squirt water through the tail of the pouch, lift it so the water swishes around the stoma and barrier, then drain it out.
- If you use a two-piece system, you can un-snap the pouch afterward and use soft tissue to clean the flange.
- Many people find this method allows them to wear the same appliance for 5-7 days.

2. Consider colostomy irrigation:
- Daily irrigation, which takes about an hour, can significantly reduce output for 24 hours, helping to prevent pancaking, odor, and leaks.
- Supplies for irrigation are available online, and there are many YouTube tutorials to help you learn the process.
- After successful irrigation, some people find they can wear a small stoma cap instead of a full pouch.

3. Use barrier rings:
- Moldable rings placed under the wafer can help block seepage if stool gets under the flange. Some people find them helpful, while others experience detachment issues.

4. Lubricate the pouch interior:
- Add a few drops of baby oil or another type of oil into the empty pouch, then blow a little air in and massage it around. This helps the stool slide down instead of pancaking.

5. Adjust diet and stool consistency:
- Aim for an output that is just liquid enough to keep moving or has a pudding-like thickness.
- Some strategies include:
- Limiting coffee to one cup daily.
- Drinking plenty of juices, such as apple or prune juice, about 5-6 pints a day.
- Eating high-fiber porridge oats with mixed fruit and walnuts.
- Taking 25 grams of macrogol (polyethylene glycol) in juice each day.
- Using an over-the-counter stool softener once or twice daily.

6. Product preferences:
- Hollister two-piece systems are praised for their build quality and ability to last 5-7 days when combined with rinsing and maintaining proper stool consistency.
