Stoma complications - Considering reversal surgery

This topic is about someone who has been dealing with persistent skin issues and leakage around their stoma, which has led them to consider reversing their ileostomy. They initially had a loop ileostomy after rectal cancer surgery, but complications like a parastomal hernia have made managing the stoma increasingly difficult. Here are some insights and advice shared by others:

1. Skin-prep & Adhesion:
- Use a non-alcohol skin prep along with Coloplast Adhesive Spray to create a tacky surface for better adhesion.
- Apply a small bead of Stomahesive around the stoma and use a Coloplast moldable ring. Warming the wafer by hand for 15 minutes can help improve the bond.

2. Accessory Strips & Convexity:
- Coloplast Brava half-moon or accordion barrier strips can help secure wafers over hernia bulges. Ensure the skin is shaved and oil-free for better adhesion.

3. Alternate Pouching Systems:
- Nu-Hope Laboratories offers a non-adhesive O-ring barrier held by a belt, allowing the skin to breathe while healing. They provide customized samples after consultation.
- Consider custom or DIY rigid baseplates made from heat-molded plastic attached to elastic belts. This setup allows appliances to adhere to the plate instead of the skin.
- Liberator Medical can provide samples from multiple manufacturers and offer product guidance.

4. Powders / Pastes / Creams:
- Hollister Karaya 5 and Medline silver powder can help reduce moisture and bioburden, though they require frequent re-application.
- Desitin Maximum Strength zinc-oxide diaper cream or Zincofax can be applied to raw skin to calm inflammation within 24 hours. Taking breaks from wafers can aid healing.

5. Alternative Surgical Options:
- Consider the Barnett Continent Intestinal Reservoir (BCIR) as a non-adhesive, continent alternative if a conventional reversal is not suitable.

6. Imaging Preparation:
- For CT scans, using IV contrast instead of oral prep can help avoid uncontrollable effluent. Confirm this option with your radiologist or surgeon.

7. Hernia Support:
- A hernia belt, such as those from Nu-Hope, can help reduce the bulge and improve wafer contact. Early fitting is recommended.

8. Decision-making:
- It's important to conduct a thorough risk-benefit analysis before deciding on a reversal, as outcomes can vary. Discuss options with an experienced colorectal surgeon and WOC nurse.

9. Emotional Support:
- Many community members express empathy and encourage perseverance, offering support and well-wishes for a successful surgery and recovery.
