Leaking Issues with Peristomal Hernia Despite Using a Hernia Belt

This topic is about dealing with leaks from an ostomy pouch when you have a peristomal hernia. The person sharing their experience has had an ileostomy since 1981 and only started experiencing leaks after developing a hernia. They are using a Hollister flat one-piece pouch and a hernia belt but are still facing issues, especially at night. Here are some suggestions and insights that might help:

1. Consider trying a flange with convexity. Many people have found that using convex products can stop nighttime leaks. You can request free samples from different manufacturers like Hollister, Coloplast, and ConvaTec to find the best fit for you.

2. Combine convexity with barrier or "donut" rings, or use convex rings. These can help pull the stoma out and create a tighter seal.

3. If your stoma is flush or retracting, you might want to add stoma paste or adhesive around the stoma and on the flange for extra security. This can help extend the wear time to about three days.

4. If you use an overnight drainage bag, make sure the tubing and bag are well supported. This can prevent tugging during the night, which might break the seal.

5. It might be helpful to re-evaluate your entire system with a qualified Stomal (WOC) Nurse. A professional fitting and possibly switching brands or product styles can be more effective in eliminating chronic leaks than relying solely on advice from the product manufacturer.

6. Continue using a hernia belt, but ensure it is not too tight or causing the pouch to shift. Some belts have built-in openings for convex wafers that help maintain even pressure.
