Advice Needed for Bleeding Around Stoma with Bag Changes

The topic at hand is about dealing with small bleeding spots around the stoma when changing an ostomy bag. This can be concerning, so here are some reasons why it might be happening and some practical tips to help prevent it:

1. Wafer Opening and Paste:
- The opening in the wafer might be too large, allowing waste to irritate the skin. Try cutting the wafer closer to the stoma.
- Use a thin layer of stoma paste around the opening to create a better seal.

2. Picture-Frame Skin Protection Method:
- Use four 2-inch strips of Stomahesive around the stoma like a picture frame, as close to the base as possible.
- Place a Sur-Fit wafer over these strips.
- Wrap a folded 4-inch Stomahesive Strip around the stoma base over the wafer and tape the wafer edge for a secure fit lasting 5–7 days.

3. Normal Vascular Bleeding:
- A small amount of bleeding can be normal since the stoma is made of tissue rich in blood vessels. Minor trauma from wiping or the wafer edge can cause this and is usually not serious.

4. Expect Improvement Over Time:
- Some people find that early bleeding decreases as the stoma and surrounding skin become tougher.

5. Alternatives to Cutting:
- Consider using a Convatec Moldable Skin Barrier that fits snugly around the stoma, leaving no skin exposed.
- Convex wafers can also help improve the seal and keep the skin clean and dry.

6. Powder and Ring Products for Irritated Skin:
- Products like "C" Stoma Powder, Adapt Stoma Powder, and Adapt Barrier Rings can absorb moisture and fill uneven skin. Seal with an adhesive wipe before applying the wafer.
- Some people use a sequence of powder, adhesive wipe, then wafer for extra protection.

7. Infection Possibility:
- If irritation continues, it might be a mild bacterial infection. Consider using Adept (Adapt) Paste with an antibacterial solution during each change.

8. Product Endorsements:
- Convatec bags with Stomahesive backing are recommended for their skin-healing properties and comfort.
