This topic is about managing skin irritation and obtaining supplies after a permanent colostomy, specifically for someone living in Fiji. The person had their surgery in 2012 and faces challenges due to the lack of local ostomy products, which need to be imported at a high cost. They have found that the Convatec two-piece system works best for them and regularly practice irrigation, which helps with their routine. However, they are dealing with severe skin irritation around the stoma, which affects the adhesion of the wafer. Here are some helpful tips and advice:

1. Skin Protection
- Apply fresh aloe vera gel directly to the irritated skin, ensuring to remove the sharp edges of the leaf first. Follow this with a barrier spray.
- Once the skin is healed, use no-sting barrier wipes like Clinimed “LBF Barrier Wipes” to protect the skin.

2. Wafer Adhesion and Support
- Make sure the area around the stoma is thoroughly washed and dried after each irrigation session before applying any supplies.
- Consider creating a custom rigid support plate to help with hernia pull and keep the wafer stable. You can use a ½-inch plastic skirting board, cut to size with a central hole for the stoma. Attach Velcro on the front for an elastic strap or belt, and use double-sided carpet tape on the back to secure it to the wafer.
- Be prepared to try several prototypes to find a comfortable fit.
- Alternatively, look for a commercial rigid belt with a comfort-lined cavity that sits over the stoma and fastens with Velcro around the torso.

3. Product Sourcing and Financial Help
- Reach out to as many ostomy manufacturers as possible to request multi-week sample packs, allowing you to test products without immediate costs.
- Seek assistance from charitable organizations for ongoing supply needs. One example is "Hand of Hope."

4. Lifestyle Tip
- Continue with or learn more about irrigation, as it simplifies daily life and reduces the need for appliances. This practice is already part of the routine and is highly recommended.
