Leaking Pouches - Seeking Advice for Persistent Colostomy Issues

This topic is about someone who has been experiencing persistent night-time leaks with their Coloplast 1-piece flat barrier system after initially having success for over four months. They are seeking advice and solutions to address this issue. Here are some helpful insights and suggestions:

1. Consider returning to Hollister products, as some users have found that switching back eliminated leaks. Specific products mentioned include the Hollister barrier ring 7805 and flange 14803.

2. Try using a 2-piece system instead of a 1-piece. This can make alignment easier, allow for a double-ring technique, and simplify bag changes. Both Coloplast and Hollister offer free samples.

3. Combine different brands, such as using Hollister barrier rings with a Coloplast flange or bag. Adding Coloplast elastic barrier strips can help prevent the flange edge from peeling up.

4. Use a convex wafer or collar, like the Salts “Collar Seal” part DC29, to direct output forward and reduce the risk of leaks.

5. Regularly check the size of your stoma, as it may shrink over time, leaving the barrier hole too large.

6. Strengthen the outer seal by applying Coloplast 120700 elastic strips, Brava 032155 6 × 6 sheets (with the center removed), or 3M Micropore 1" tape around the flange.

7. Enhance adhesion by warming barriers and rings under your arm or with a heat pad for several minutes. Press and massage the wafer into place, then wear a Nu-Hope belt or Nu-Hope Cool Comfort belt for an hour.

8. Use two barrier rings for a "double seal": one under the wafer against the skin and one on top around the stoma before attaching the bag. Wear gloves when handling the rings, as they are very sticky.

9. Some users prefer a Hollister 2-piece system with Coloplast moldable rings, reporting up to 7-day wear time using a heat-set method in bed.

10. Protect your skin with products like 3M Cavilon Barrier Film to prevent redness and irritation. Apply skin-prep first, followed by the Coloplast Protective Seal, flange, and bag.

11. Pay attention to the consistency of your output. Thick stool can pool at the stoma and lift the ring. Adjusting with Miralax or fiber can help keep output thinner and flowing down.

12. For cleaning, use fragrance-free and lotion-free soap, such as Cetaphil, and ensure your skin is completely dry before applying the barrier.

13. Consider accessory solutions like the IGGIE clip (a small binder clip) over the Velcro drain to prevent the tail from opening, and 3M Micropore tape as an inexpensive backup seal.

14. When handling barrier rings, warm them, cut and stretch gently, and place them starting at the bottom of the stoma. Overlapping or leaving a gap is okay, and wider rings are available for problem areas.

15. Watch for flange edge wear from clothing, and use elastic strips or tape to stop lifting.

16. Don't be discouraged. Experiment systematically with different options like convexity, ring types, belts, and brands, and revisit the last set-up that worked well for you.
