Pouching Issues - Bulge Above, Dip Below

This topic is about managing leaks from an ostomy pouch system when dealing with an uneven abdomen. The person experiences a small bulge above the stoma and a dip below it, which causes the flange to curve and leads to leaks. Here are some helpful tips and advice for managing this situation:

1. Consider trying a convex system again:
- Hollister convex 2-piece systems and sticky barrier rings are often effective for recessed stomas.
- A soft convex Coloplast SenSura Mio one-piece drainable pouch, which is made from pliable polyethylene, has helped others with similar issues.

2. Switch from a 2-piece to a 1-piece pouch:
- The rigid plastic coupling on a 2-piece system can create pressure points. A flat or soft-convex one-piece pouch, like the Hollister one-piece with a barrier ring or Coloplast SenSura Mio, might provide a better seal on an uneven abdomen.

3. Use stoma paste to fill the dip or folds:
- Coloplast Ostomy Paste and Dansac Soft Paste are recommended for creating a tight seal. They can fill deeper cavities and folds, though they might cause mild stinging on irritated skin and can be messy to remove.
- Some users have experienced 4–7 leak-free days using paste, which can be used with or instead of barrier rings.

4. Modify the barrier ring:
- Adjust the ring by tearing, stretching, or building it up only where extra thickness is needed, particularly on the bottom side.
- Using half a ring at the dip might be effective.
- Warm the ring or flange with your hands for about 5 minutes after application to improve adhesion.

5. Pay attention to technique:
- Apply the system while standing to ensure the abdomen is in its functional shape.
- Cutting the hole in the wafer or adhesive accurately is crucial.

6. General encouragement:
- Bodies can change throughout the day, so keep trying different products and configurations from brands like Hollister, Coloplast, Dansac, and Eakin Stomawrap until you find a reliable seal.
