Stoma Retraction and Skin Irritation Concerns

This topic is about a new ostomate from South Australia who is experiencing some concerns after being discharged from the hospital. She noticed that her stoma, which initially protruded like "one small plum and one small grape," now seems to have retracted to be flush with or slightly below skin level. She also observed a pronounced red ring where the wafer seals and is unsure if the wafer opening is cut correctly, if she's pressing the pouch on too hard, or if her use of barrier cream and stoma powder is contributing to the issue. She is hoping to avoid an urgent hospital visit and wants to know if these changes are normal.

Here are some helpful insights and advice:

1. Continue using a convex wafer or a convex one-piece bag. You might also consider adding a convex barrier ring to help push the stoma forward and minimize leaks.

2. The red ring could be caused by:
- Adhesive irritation.
- Output leaking onto the skin.
- The wafer hole being cut too tight.
If the ring disappears after washing and isn’t raw, it is usually harmless.

3. Firmly pressing the wafer for several minutes when applying it is standard practice and is unlikely to be the cause of the redness.

4. Stoma shrinkage is normal in the early postoperative period, but it should ideally remain above skin level. If the stoma stays at skin level or retracts, it might need evaluation.

5. Contact your stoma nurse and possibly the surgeon. A revision may be necessary if a convex system cannot maintain a seal.

6. Be aware that stoma size and shape can change repeatedly during the first weeks and months, so ongoing assessment and appliance adjustment are expected.
