Tightness Around Ostomy Appliance When Standing Up

This topic is about someone who, eight weeks after having an ileostomy, is experiencing uncomfortable and sometimes painful tightness around their ostomy appliance whenever they stand up. They notice that their skin looks healthy when they change the appliance, and they feel relief when the appliance is off or after taking ibuprofen. 1. The person has checked that the wafer opening is not too small or too wide. They suspect that healing abdominal muscles might be causing the tightness, but they plan to consult their surgeon to rule out a parastomal hernia.

2. They are considering using the muscle relaxant methocarbamol to see if it helps.

3. It's important to remember that the size of a stoma can change. If the wafer hole is too tight, it can cause a feeling of tightness or pulling.

4. To address this, try cutting a larger hole in the wafer and using a moldable barrier ring to protect the skin around the stoma. Some recommended products include Coloplast Brava 2-inch rings, Hollister 4 mm moldable barrier, and Coloplast 2.3 mm Brava barrier.

5. There are different ways to use barrier rings:
- Place one ring around the stoma before applying the wafer.
- Some people use a "double seal" by placing one ring under and another on top of the wafer for extra protection.

6. Leave about 0.2 inches (a little less than ¼ inch) of space between the stoma and the wafer opening. This allows for changes in stoma size while preventing leaks.

7. Full healing of abdominal muscles can take more than three months, and some muscle tightness is normal during recovery.

8. A parastomal hernia might feel like pulling. To prevent this, avoid heavy lifting and consider using an ostomy hernia support belt.

9. Wearing a hernia or support belt continuously can work well with a larger wafer opening and a barrier ring, helping to prevent pressure damage.

10. If the tightness continues, it's a good idea to have a surgeon check for any overly tight suturing or other structural issues. Gentle stretching over time might help resolve tightness related to stitches.
