Dealing with Rectal Stump Issues and Relief After Diarrhea Attack

This topic is about dealing with issues related to a rectal stump after a Hartmann’s procedure, particularly focusing on blockage, pain, and relief following an episode of acute diarrhea. Here are some insights and advice shared by individuals who have experienced similar situations:

- After a Hartmann’s procedure, a small rectal stump can fill with thick mucus that is difficult to expel, especially if the pelvic floor and sphincter muscles are tight. This can lead to "phantom pains" in the anus.

- Physiotherapy might not always help in relaxing the pelvic floor, and while Botox injections can be suggested, they are not always accepted by patients.

- Small episodes of diarrhea can occur, releasing foul-smelling mucus. While enemas can provide relief, they should be used cautiously to avoid the risk of perforating the rectal stump.

- Severe abdominal pain can sometimes be linked to infections, and in some cases, a sudden large diarrhea episode can relieve the blockage and reduce pain significantly.

- Heavy bleeding during the emptying of the rectal stump can occur, especially if the person is on anticoagulant therapy.

- If abdominal pain persists and resembles past bowel-blockage pain, further investigations like upper endoscopy and cardiac imaging might be necessary, though they may require general anesthesia due to limited IV access.

- Another person with a similar condition shared their experience of initial small mucus discharges, followed by a period of no output, and then expulsion of dried mucus. They experienced bleeding without pain, and a sigmoidoscopy showed only inflammation.

- Increased mucus production may require frequent drainage, and minor leakage at night can be managed with nighttime pads.

- Some choose not to undergo further surgery like a proctectomy with anus closure, also known as "Barbie Butt."

- Medications like Mesalamine can be prescribed, but enema formulations are often avoided to reduce the risk of perforation.

- It's common to experience rectal stump inflammation and intermittent bleeding, which can often be monitored conservatively if there is minimal pain.

- Enemas and other treatments should only be used under medical guidance due to the risk of perforation in a short, blind rectal stump.

- Maintaining a sense of humor, resilience, and open communication with healthcare providers and fellow ostomates can be crucial in managing both the physical and emotional challenges of living with a rectal stump.
