Help Needed: Solutions for Pancaking with Colostomy

This topic is about finding solutions for a common issue faced by those with a colostomy, known as "pancaking." Pancaking occurs when stool collects around the stoma instead of dropping into the pouch, which can be quite frustrating. Here are some helpful tips and advice to manage this issue:

1. Hydration & Electrolytes
- Make sure to drink plenty of fluids, around 2 liters or more each day, to keep your output softer and less sticky.
- Consider adding sugar-free or low-sugar sports drinks to your routine to maintain electrolyte balance. Some people find that drinking up to 120 ounces daily helps.

2. Diet Adjustments
- Try to reduce or monitor your intake of starchy foods like bread, rice, and potatoes, as well as foods that can make stool pasty, such as sweet potatoes and squash soup.
- Keeping a food diary can help you identify any personal trigger foods.
- If your output is too thick, you might try loosening it with foods or drinks like apples, coffee, or probiotics. Avoid or cut back on thickening agents like loperamide, and some people use Mylanta to thin their output.

3. Maintain Air Space in the Pouch
- Allow a small amount of air to remain in the pouch so gravity can help pull the output downward. You can temporarily cover the filter on closed-end bags until some air builds up, then uncover it.
- Avoid wearing very tight clothing across the stoma. Loosen waistbands or push them outward if output starts to collect.

4. Lubrication Inside the Pouch
- Add a few drops of baby oil inside the pouch to create a slick surface.
- Use commercial lubricants like Hollister Lubricating Deodorant Packet #78501 or other ostomy lubricants.
- For drainable pouches, pour half to three-quarters of a cup of water into the bag, swish it around, then empty it. This helps clear existing pancaking and leaves a wet, slippery surface.

5. Adding Weight to a Closed-End Bag
- Place a small object, like a folded tissue or a couple of cotton balls dampened with essential oil, in the bottom of the bag to help gravity do its job.

6. Early Intervention & Cleaning
- With a two-piece system, detach the pouch and wipe away stool before it creeps under the wafer. Change the bag at the first sign of pancaking.
- Be cautious, as frequent cleaning under the flange can sometimes lead to leaks, so try to balance preventive cleaning with maintaining the seal's integrity.

7. General Tips
- Keep the pouch interior lightly oiled and aired at all times.
- Swishing water in the pouch is the drainable-bag equivalent of the "remove and wipe" technique used with closed-end bags.

These tips and adjustments can help manage pancaking and make living with a colostomy more comfortable.
