Switching from Colostomy to Ileostomy for Colovaginal Fistula

Switching from a colostomy to an ileostomy can be a significant decision, especially when dealing with a colovaginal fistula that causes stool leakage through the vagina. Here are some helpful insights and advice to consider if you're facing this situation:

1. Talk to your surgeon to understand why an ileostomy is being suggested. Sometimes, removing just the affected part of the bowel can solve the issue without needing to switch to an ileostomy. If you're unsure, don't hesitate to seek a second opinion.

2. Check the position of your current stoma. If your colostomy is placed below the fistula, it might not be effectively diverting stool away. Moving the colostomy higher might stop the leakage without needing an ileostomy.

3. Be aware that an ileostomy will stop formed stool but might not completely prevent rectal mucus discharge. This means the fistula might still cause some vaginal discharge.

4. Have a conversation with a stoma nurse. They can connect you with others who have had similar surgeries and might suggest less drastic options.

5. Reach out to the Ileostomy and Internal Pouch Association (I-I-PA) for peer support and more information about your surgical options.

6. Learn from others' experiences. For instance, one person managed a low colovaginal fistula with a colostomy, which stopped the leakage, although there was still a small amount of clear discharge because the fistula couldn't be closed surgically.

7. Remember that switching to an ileostomy is a major change. Carefully consider the benefits, potential risks, and how it might affect your quality of life before making a decision.
