Struggling with Ostomy Bag Changes and Skin Irritation

This topic is about someone who is dealing with a flat stoma, severe skin irritation, and frequent issues with their ostomy appliance not staying in place. They had emergency surgery and are now facing challenges with their skin and the appliances they use. Here are some helpful insights and advice shared by others who have faced similar issues:

1. Consult a wound-ostomy specialist or dermatologist. It's important to treat the raw, inflamed skin before trying to find the right appliance. The irritation could be due to an allergy, fungal infection, or bacterial infection. Some have found medications like Nystatin to be effective.

2. Avoid using strong adhesives on damaged skin. Focus on healing the skin first before trying products with strong adhesives.

3. Consider a soothing cleansing routine:
- Wash the area around the stoma in the shower using Head & Shoulders Classic Clean/Classic Blend shampoo, which contains anti-fungal zinc pyrithione.
- Make sure the area is completely dry; avoid using oils.
- Lightly apply Butt Paste diaper cream and wipe off any excess.
- Dust the area with Karaya 5 powder before applying the wafer.

4. Use an ostomy barrier ring to protect raw areas and help improve the seal while the skin is healing.

5. Check for any material allergies. If you notice redness or irritation within 24 hours of using a new brand or wafer, consider switching brands.

6. Ensure the appliance fits properly and is flexible, especially with a flat stoma:
- Some have resolved chronic leaks by switching to a more flexible pouching system after consulting with an ostomy nurse.
- Press the flange firmly to the skin with warm fingers or thumbs for at least 60 seconds to activate the adhesive.
- Use Dermabond or similar skin adhesive products over irritated patches until they heal, and change the pouch as soon as output undermines the seal.

7. Remember that emotional support is important. Skin problems are common in the early stages, but healing will improve the wear time and comfort of the pouch. Stay patient and continue seeking professional help.
