Dealing with Ileostomy Bag Leaks: Seeking Advice

This topic is about dealing with frequent seal leaks in an ileostomy bag, which can be quite frustrating. The person sharing their experience has had an ileostomy since 2010 due to the removal of their colon. They have been facing issues with the seal of their ostomy bag failing, sometimes even while they are asleep or during simple activities like playing cards or watching TV. Normally, they change their pouching system three times a week when there are no leaks. They are seeking advice to prevent these unpredictable leaks.

Here are some helpful tips and insights shared by others:

1. Investigate used flanges to identify where leaks occur. Check if leaks happen in the same spot or if the pouch coupling has come undone.

2. Ensure the skin around the stoma is warm and dry before applying the flange. You can use a hair dryer to pre-heat the flange and make sure the peristomal skin is completely dry.

3. Avoid putting pressure on the pouch by steering clear of tight belts, waistbands, or sitting for long periods, especially after meals.

4. Make sure the stoma opening is correctly sized, leaving only a small clearance of 1 mm to 1 cm to prevent seepage under the wafer.

5. Use effective site preparation and sealing products. Apply a stoma paste, like Coloplast Paste Strips, around the hole and roll it into a continuous ring to eliminate gaps. A Cavilon skin barrier can be applied before attaching the wafer.

6. After applying the wafer, use snug clothing or gentle hand pressure to help the wafer and paste bond. Some people tape the wafer edges with 3M Transfer Tape for about 12 hours to secure the seal while it warms to body temperature.

7. Keep tape handy for extra support during activities like bending, exercise, or in hot weather when moisture can weaken adhesion.

8. For two-piece systems, ensure the pouch is firmly attached to the flange by double-checking that the locking mechanism is fully engaged.

9. If skin folds, weight, or loose skin are affecting adhesion, consider consulting an ostomy nurse for personalized solutions.

Despite facing additional health challenges, such as needing to self-catheterize due to an enlarged prostate and planning for future dialysis, the person continues to enjoy activities like driving and playing poker.
