Seeking Insight on Hernia and Stoma Relocation Surgery Experience

This topic is about the experiences and outcomes of undergoing major surgery for a large parastomal hernia, which includes relocating the stoma. The person sharing their story has had an ileostomy for 35 years and is dealing with a significant hernia that has caused several bowel obstructions. They are considering surgery but are concerned about the risks, especially due to their condition, myasthenia gravis, which can complicate anesthesia. They are seeking advice from others who have been through similar surgeries. Here are some insights and advice shared by others:

1. If you suspect a hernia:
- A hernia might look like a bulge around the stoma, similar to a "pitcher’s mound." As it grows, the stoma may appear flatter.
- It's important to have any suspected hernia checked by a colorectal surgeon quickly. Unmanaged hernias can lead to bowel protrusion and painful obstructions that might require emergency surgery.

2. Emergency repair versus planned relocation:
- One person shared their experience of needing emergency surgery when their intestine pushed through the hernia. They were advised that if another hernia appeared, stoma relocation would be necessary. They emphasized that early, planned surgery can prevent emergencies.

3. Successful stoma relocation:
- A 67-year-old who works 36 hours a week had their stoma moved from the left to the right side.
- They took about two weeks off work to recover.
- Following post-op lifting and activity restrictions is crucial. They admitted to being a bit lax, which might have led to a small new bulge.
- Benefits included easier pouch care and no recurrence of the hernia to its previous severity.
- They can eat a wide variety of foods without issues but suggest others monitor their own diet tolerance.

4. Preventing leaks after relocation or repair:
- Hypafix Tape (BSN Medical) is recommended.
- Apply it around the adhesive border in a box shape after placing the wafer/flange and before attaching the pouch.
- This method reportedly reduces flange edge lifting and virtually eliminates leaks without adding thickness or changing the pouching routine.

5. General advice from multiple people:
- Get any parastomal bulge or change evaluated early.
- Elective relocation surgery can improve security and simplify care if post-operative restrictions are followed.
- Using adhesive products like Hypafix Tape can help maintain a reliable seal after surgery.
