Is This Pancaking? Changing an Appliance in an Emergency

The topic at hand is about a situation where someone had to change their ostomy appliance unexpectedly and wondered if they were experiencing something called "pancaking." Pancaking is a common issue for people with ostomies, and it involves the output clustering around the stoma instead of dropping into the pouch. This can cause a thick build-up that might compromise the barrier seal and lead to leaks. Here are some insights and tips to help manage and prevent pancaking:

1. Pancaking typically looks and feels very dense, similar to "pancake make-up." It can sometimes be difficult to remove from the skin or wafer, requiring a fingernail or scraper.

2. Common causes of pancaking include:
- Thick or sluggish stool consistency.
- A flush or non-protruding stoma that allows output to spread under the wafer.

3. To prevent and manage pancaking, consider the following tips:
- Keep the stool slightly looser by drinking more fluids. If your doctor approves, you might use an osmotic laxative like Miralax (polyethylene glycol). Many find that a daily dose helps the output flow more easily, especially in the morning.
- Experiment with the wafer opening by cutting the hole slightly larger or exactly to the stoma size. Different people find success with different sizes.
- Build a small "collar" with extra stoma paste to guide the output into the bag, which can be particularly helpful for flat stomas.
- If you notice pooling at the top of the pouch, gently push on the outside of the pouch to direct the stool downward.
- Change the pouching system daily or carry spare supplies so you can quickly swap everything out if pancaking threatens a leak.
