CPAP and Ileostomy - Sleep Apnea Issues?

This topic is about managing the use of a CPAP machine for sleep apnea when you have an ileostomy. The person who started the discussion has an ileostomy and has recently begun using a CPAP machine. They are experiencing some issues and are curious if others with an ostomy have faced similar challenges. Here are some helpful tips and insights shared by others:

1. Clearly describe the specific problem you are facing. This will help others provide more targeted advice. Many people find CPAP machines very helpful for improving daytime energy, even if there are some minor annoyances at night.

2. Swallowing air while using a CPAP, especially if you breathe through your mouth, can lead to excess gas. This can inflate the ostomy pouch and might even cause it to detach.

3. To reduce swallowing air (aerophagia), consider using a chin strap to keep your mouth closed:
- Hospitals often provide standard adjustable chin straps.
- You can make a DIY strap using a folded cotton pillowcase, about 2 inches wide, fastened with Velcro. Using non-elastic fabric can help prevent your jaw from dropping.
- To avoid rashes or chafing, you can pad the strap with thin neoprene or use a custom plaster-of-paris chin cup lined with soft tissue.

4. Adding a few drops of Olbas Oil to the CPAP humidifier water, or further up the hose if you use a filter, can help clear your sinuses and make nasal breathing easier.

5. For additional comfort and better air management:
- Use a small Velcro tie to secure the hose to the center of your chin, which helps keep the mask stable.
- Sleeping on an incline, like in a "hammock position," can reduce reflux and involuntary air swallowing.

6. For managing your ostomy at night:
- Consider switching from a standard pouch to an irrigation sleeve or a large-capacity bag while sleeping. This can significantly reduce the risk of a gas-filled pouch detaching.
- A semi-permanent two-piece baseplate with a 90-degree waste bend can direct output downward and minimize pancaking. Using medical adhesive spray can prevent sliding.
- Reusable irrigation sleeves, which come in boxes of about 12, can be rinsed nightly and reused for about a month each, making a single box last roughly a year.

7. Regarding air quality, inserting an inline CPAP filter can block outdoor pollutants. You might need to slightly increase the machine's pressure to compensate. If you use scented oils like Olbas, add them after the filter so the aroma isn't removed.
