Help with Ileostomy Leaking Issues and Skin Concerns

Hi there!

I had an ileostomy on June 25, 2024.

I am using a two-set system (Hollister) convex flange with a bag. Then I had some skin issues where my stoma nurse said I should use a flat flange.

I had lots of leaking issues (two where the *hit literally went everywhere) even with extenders placed on the outside of the adhesive of the flat flange. I changed one after showering at 2 a.m. only to have another blowout at 4 a.m.!

I went back to the convex flange but am worried about my skin. Any advice for a newbie?

How far out from your skin does your stoma protrude? It should be at least 3/4 of an inch (19 mm) or more. Too close to your skin level will cause nothing but trouble. As time goes by, your stoma will shrink slightly.

If your leak problems continue, consult with a surgeon about revision surgery. Your stoma can easily be pulled out.

Hello everyone!!!

It has been almost a year since joining this site. I just wanted to share. I know a lot of people get concerned about meeting someone on here. I'm not a lifer membership and I had someone initiate to take me out. I've been divorced since 2009. He lives in Chicago. I ignored the first time thinking he was joking. I live in Milwaukee. So a month later he sent me a message through this site saying he wanted to meet me. That was in April this year. I must say it was the best thing that could happen to me. We talk almost every day. We have so much in common. He comes up all the time to spend the day or weekend together. It really does make a difference when someone has an ostomy like you. Such understanding and we always have something to talk about. I want to share because sometimes people think it's hard to meet someone or they say this site doesn't work. I'm a testimony that this site does work. We are talking marriage this time next year. I will keep you posted.

Hi SS,

I think we need a bit more information to figure out what's going on. Why exactly is your barrier failing? Do you have uneven skin, or is your stoma not sticking out very far? What did your ostomy nurse tell you was going on? Not sure how a convex barrier could cause skin issues that a flat barrier wouldn't, so we need to unpack this a bit. Have you ever caught a leak before it became a gusher? If so, where did the leak start? Give us some more info and we'll be better able to help you out.

When you are cleaning your skin area, make sure you are not using soap with oils in it. DIAL or IVORY works best for me. You will get lots of suggestions from fellow Ostomy folks… best of luck.

In addition to all the other good advice, make sure your skin is completely dry. The use of a blow dryer set on low is helpful. If you are using the stoma powder, use only as needed and just a light dusting followed by the skin prep wipes.

All good advice. If you haven't tried a barrier ring, that might just be all you need to solve your problem. As you live in Canada (welcome from another BCer!), you have access to Salts products. I recommend their aloe barrier rings. They were a game changer for me.

My stoma does stick out and is not flush to the skin, so I don't think that is the problem. But thank you for your answer!

I have a hernia that looks like a third boob! So my skin around the stoma is more like a hill rather than flat.

I've gone back to the convex flange for about 2 weeks now and have had no leaks (knock on wood).

The leaks were always on the right, on the same side where my skin wound is.

Thank you so much for your reply.

I use either saline or just water to clean the skin, then Cavilon to protect the skin. I let that dry before attaching the barrier ring and flange.
