Need tips for using stoma collars with loop ileostomy?

This topic is about finding a solution for using Dermacol stoma collars with a loop ileostomy that has two openings. The main challenge is to protect the skin around the stoma without blocking the secondary opening, which can lead to discomfort and gas build-up. Here are some helpful tips and insights:

1. Consult with an ostomy nurse. They can examine the loop ileostomy in person and recommend suitable products or custom modifications.

2. Avoid blocking the second opening. A loop ileostomy releases mucus and small amounts of gas, so sealing it can cause ballooning, leaks, and skin irritation.

3. Consider using Salts mouldable seals, such as putty or rings, placed around the second opening rather than over it, and then apply the appliance as usual.

4. Request samples of the next size up in Dermacol collars. A larger size might clear the second hole and address both the gas build-up and skin protection issues.

5. Try custom modifications by punching or cutting a relief hole in the collar’s adhesive. If necessary, extend the adhesive area with medical adhesive spray to ensure it stays in place.

6. For a DIY approach, create a wider plastic baseplate or extender. Secure it with medical adhesive spray, or construct one using sections of plumbing waste-pipe until the fit is just right.

7. If collars continue to leak, consider switching to mouldable barrier rings. Options include:
- ConvaTec mouldable rings (specific name not recalled, but available through UK stoma services).
- Hollister Adapt barrier rings.
- Request samples from delivery company nurses for Convatec or Hollister products.

8. Barrier rings can be stacked or shaped to form a "gutter" that protects the sore area while keeping the loop opening free.

9. If problems like bleeding or recurring leaks persist, seek a professional review to rule out any complications.
