Ileostomy retraction - Tips for fitting flange with pancaking issues

This topic is about dealing with the challenges of an ileostomy that sometimes retracts, especially when the output "pancakes" against the flange. This can make it tricky to get a good fit with the flange, leading to leaks. The retraction seems to happen more after heavier meals or changes in food and drink. Although the stoma usually returns to its normal position, it can be frustrating to maintain a reliable seal while still enjoying meals. Here are some helpful tips and insights:

1. To prevent pancaking and guide output away from the stoma:
- Consider using a Salts Healthcare Dermacol Stoma Collar, which lines the stoma and helps keep output off the skin and moving downward.
- A DIY option is to use a baseplate with a 90-degree waste-bend elbow or a sleeve to channel output downward.

2. Use a convex system when the stoma retracts:
- Switching to a convex flange or a convex one-piece pouch can help the stoma protrude and prevent leaks, especially on uneven abdomens or with naturally flush stomas.

3. Allow some air in the pouch:
- Completely air-free appliances can encourage pancaking. Letting a little air in helps the output drop to the bottom. Covering or disabling the filter during the day can assist with this.

4. Control excess air without breaking the seal:
- The Osto-EZ-Vent, which can be attached to any pouch, allows you to "burp" the bag on demand. This helps prevent ballooning while still leaving enough air to deter pancaking. Be prepared for a small amount of effluent when venting.

5. Consider meal size:
- Large or heavy meals can create more gas and thicker output, increasing the chances of pancaking and ballooning. Smaller, more frequent meals might help reduce these issues.

6. Be aware that temperature or external pressure can also cause temporary retraction, so plan your gear (like using convexity or collars) accordingly.
