Struggling with Ileostomy Appliance Leaks

This topic is about dealing with persistent leaks around an ileostomy, which are caused by indentations or "dips" in the skin that have developed after weight gain. The person experiencing this issue used to have a reliable 7-day wear time for their ostomy appliance but now needs to change it several times a day. They are looking for practical solutions to regain longer wear time while they work on losing weight. Here are some helpful suggestions:

1. Skin preparation and cleaning:
- Wash the skin around the stoma with a non-perfumed soap, like Ivory, and make sure it is completely dry.
- Use adhesive remover wipes before applying a new wafer. If the skin is irritated or weepy, it needs to heal for the barrier to stick properly.

2. Filling the "dips":

3. Alternative rings and seals:

- Sure Seal Rings act as a thin, clear, oxygen-permeable "second skin" over any barrier, reportedly doubling wear time in heat, heavy sweating, showering, and lawn work.

4. Convexity and flange choice:
- Switching to a convex flange, such as Hollister convex, Coloplast Sensura Click Convex, or Convatec Moldable Convex, can help press the wafer into the dips and prevent leaks.
- Wearing a snug support belt over the pouch can maintain even pressure on the flange and help keep the seal intact.

5. Adhesives and skin protection:
- Use stoma powder followed by skin-prep wipes at every change.
- New Hope Liquid Adhesive, applied to the skin and dried with a hair dryer before attaching a warmed barrier ring, can extend wear time to 5-7 days, even in hot tubs.

6. Output management:
- Thicken watery output with dietary changes, such as eating marshmallows, peanut butter, bananas, or rice.
- Eakin Gel Thickener sachets placed inside the pouch can turn liquid output into a gel, reducing leaks.

7. Heat and humidity solutions:
- In very hot climates, rings may "melt." Users report better performance with Hollister Adapt or Coloplast Sensura systems combined with Sure Seal Rings.

These strategies, which include proper skin care, using convexity, filling dips with rings or paste, using additional adhesive products, wearing a support belt, and managing output consistency, have helped many people in similar situations achieve 4-7 day wear times without leaks.
