Advice on applying Karaya powder for colostomy durability?

A person with a permanent colostomy is experiencing reduced wear-time of their wafer and is seeking advice on how to properly use these products to protect their skin and enhance adhesion.

1. Karaya / Stomahesive powder technique:
- Lightly dust the powder only on the reddened skin immediately around the stoma.
- It is okay if some powder lands on the stoma itself.
- Be sure to remove any excess powder, as too much can weaken the adhesion of the wafer.

2. Evaluate the skin problem:
- If the irritation is elsewhere under the wafer or appears as a rash, it might be a yeast infection. In this case, an antifungal powder may be more effective than Karaya.

3. Product suggestions:
- Convatec Stomahesive Powder (#25510) is similar and also available in a spray version.
- 3M Cavilon barrier products (Australian code 3344E) can be used as an alternative skin-prep, though some find that powder alone provides a stronger seal than the Cavilon spray.
- Coloplast one-piece pouches with a shallow base plate are reported to have excellent adhesion.

4. Barrier ring placement:
- Many users place the Hollister Adapt ring directly on the skin around the stoma first, then apply the wafer. This method is believed to help center the ring better and improve the seal.

5. Consult the professionals:
- A stomal therapy nurse (STN) can assess the skin condition, confirm product codes, and demonstrate the correct application techniques.

6. General adhesion experience:
- A long-term Hollister user noted that wafer adhesion has become less reliable since a packaging change. If leaks occur despite using rings, paste, and powder, consider changing the appliance more frequently until a better solution is found.
