Ileostomy Bag Leakage - Seeking Advice and Solutions

This topic is about someone who has been living with an ileostomy for six months and is suddenly experiencing frequent leaks from their ostomy bag. They are seeking advice and solutions from others who might have faced similar issues. Here are some helpful insights and suggestions:

1. **Check for Product Issues and Seek Assistance**
- Report any lot numbers to your supplier, as there might be a defect in the batch.
- Contact companies like Hollister, Coloplast, or ConvaTec for free samples and advice from their ostomy nurses.

2. **Assess Stoma and Skin Area**
- If your stoma is flush or lower on one side, consider trying a convex wafer for better fit and security.
- Use a convex wafer with an Eakin Seal or similar barrier ring, as they serve different purposes.
- Re-measure your stoma, as its size and shape can change over time.

3. **Simplify Skin Preparation**
- Use only warm water to clean healthy skin, dry thoroughly, and then apply the flange.
- Barrier wipes are only necessary when the skin is raw; they don't improve adhesion on healthy skin.
- Avoid soaps with oils, lotions, or perfumes; a fragrance-free option like Cetaphil bar soap is recommended.

4. **Enhance Adhesion Techniques**
- Ensure your skin is completely dry before applying the ring or wafer; use a hairdryer on low to warm the area and pre-heat the wafer.
- After applying, hold your hand or a heating pad over the flange for 5–30 minutes to help the adhesive bond.
- Cut the opening just big enough to leave a tiny gap, allowing the stoma to flex without output contacting the skin.

5. **Address Irritated Skin and Leakage Issues**
- For red, weepy skin, apply a liquid antacid like Maalox as a protective coat, let it dry, then dust with stoma powder and use a barrier wipe if needed.
- If you suspect a fungal rash, use an antifungal powder or cream under medical guidance.

6. **Practical Tips from Others**
- Cool the skin with a damp cloth after removing the old pouch to soothe irritation and make cleaning easier.
- Fast or reduce fluid intake briefly before changing the pouch to minimize output.
- Check the back of each removed wafer to identify where leaks start, which might indicate a need for convexity or better ring shaping.
- Be aware of any parastomal hernia development, as bulging can lift the wafer edge and cause leaks.

7. **Consider Different Brands and Products**
- Explore options like Hollister (standard and convex wafers), Coloplast pouches, ConvaTec convex systems, Eakin Seal, Cavilon spray protectant, Cetaphil bar soap, Maalox, and Duoderm as alternative seals or pastes.

By making these adjustments, many have found they can extend the wear time of their appliances and reduce unexpected leaks.
