Stoma Recedes When Lying Down - Seeking Advice for Revision Surgery

This topic is about finding non-surgical solutions for a person whose stoma retracts when lying down, leading to leaks and sleep difficulties. The person in question has a high-output ileostomy and experiences issues primarily at night, which has led to sleeping in an upright position and feeling exhausted. Here are some suggestions and insights that might help:

1. Consider using a convex wafer if not already in use. Convex wafers can help push the stoma outward, potentially reducing leaks when lying down.

2. Try sleeping on the side or in a semi-prone position rather than flat on the back. This can prevent the output from pooling around the stoma and reduce the risk of leaks. Many people with high-output stomas find this position helpful.

3. Use additional sealing products like a slim barrier ring under the wafer. This can significantly reduce edge leakage.

4. Look into sleeping equipment that keeps the upper body elevated, such as an adjustable bed or a reclining lounge chair. This allows for sleeping at an angle while still being in bed.

5. Loosen or remove the ostomy belt during sleep. The belt should be loose enough to fit two fingers underneath. A tight belt can pull on the pouching system and cause failures, especially when turning in bed. Some people use rigid protectors like Stoma Dome or StomaShield for daytime support instead.

6. Stomach-sleeping is generally risky due to the potential for bag bursts or seam splits. However, some manage it by checking the pouch for fullness or leaks immediately upon waking.

7. If non-surgical measures do not resolve the issue, consider discussing surgical revision with a colorectal surgeon. However, many people find that adjusting the pouching system and changing sleep positions can effectively address similar problems.
