Struggling with Irrigation Due to Hernias - Advice Needed

This topic is about someone who has been successfully using irrigation for their ostomy for years but is now facing challenges due to multiple hernias. They are finding it difficult to completely empty their colon during irrigation, and more output appears a couple of hours later. Adjusting the water volume hasn't helped, and surgery is considered too risky. They are looking for practical solutions to regain reliable control and are thinking about consulting an ostomy nurse.

Here are some helpful suggestions and insights:

1. Hernias don't necessarily stop you from irrigating, but you might need to tweak your technique.

2. Water infusion techniques:
- Some people find it helpful to fast or irrigate first thing in the morning, while others prefer to have a full breakfast and sip a hot drink like tea during infusion to stimulate peristalsis. Try different approaches to see what works best for you.
- Water volumes typically range from 500 cc to 1500 cc. An ostomy nurse can help you figure out the right amount based on your colon's length.

3. Physical positioning and stoma handling:
- Gently pressing or massaging the hernia or stoma area can help straighten the bowel, making it easier for water to enter and exit.
- Experiment with lifting one leg, changing your posture, or shifting your body position while infusing.

4. Post-infusion strategy:
- Eating a light snack or meal right after can help prompt any remaining residue to empty.
- Be prepared for occasional "slow movers" by wearing a Hollister mini-pouch to manage small outputs that occur later.

5. Diet tips:
- Limit very fibrous or stringy foods like fresh pineapple and celery, as they can slow flow or cause blockages. Canned alternatives might be safer.

6. General tips:
- Stay relaxed during the return phase by reading, listening to calming music, or doing something similar.
- Don't give up; with consistent experimentation, you can usually find a routine that works, even with hernias.

7. Professional help:
- It's highly recommended to consult with a WOCN/ostomy nurse for personalized advice on volume, technique, and equipment.
