Struggling with Stoma Placement - Surgeon's Mistake, Need Advice

This topic is about someone who recently had a colostomy and is facing challenges because their stoma was not properly positioned during surgery. The stoma points downward, causing the output to leak and irritate the skin. The person is also dealing with loose skin and abdominal creases, which make it difficult for the appliance to stick properly. Despite trying various products, nothing has worked for more than two days. The surgeon acknowledged the placement issue, and now the person is seeking advice on how to fix the problem and prevent leaks.

Here are some helpful suggestions and insights from others who have faced similar issues:

1. Hollister Solution: Many recommend using the Hollister New Image Skin Barrier #14803. This is a convex barrier with a funnel-cone back that fits snugly around a low or recessed stoma, helping to reduce leaks.

2. Convex System and Barrier Rings:
- Try using a convex wafer, such as the Sensura Mio Convex, along with Brava 2-inch thin barrier rings or other convex inserts to help push the stoma outward.
- Consider adding crescent-shaped barrier extensions and wearing a belt to apply gentle pressure, which can improve the stoma's protrusion.

3. "Crusting" Technique for Sore Skin:
- Apply calamine lotion to the sore skin and let it dry.
- Dust the area with stoma powder, blow off any excess, and then seal it with adhesive spray before placing the wafer.
- Warming the wafer or pouch with a heating pad for five minutes while pressing it on can enhance adhesion.

4. Surgical Revision:
- Some have found success with a formal stoma revision surgery. It's important to advocate firmly with the surgical team, as a misplaced or recessed stoma can be surgically rebuilt into a more protruding "rosebud" shape.

5. Colostomy Irrigation:
- For those who can irrigate, doing so daily or every other day can virtually stop output for over 24 hours. This prevents pancaking and allows the use of a small stoma cap instead of a full pouch, making it less critical for the barrier to stay on perfectly.
