According to their sizing directions, I am 40”, which is XL. I have never been an XL around my waist. Do these wraps run big or small?

I'd say they are pretty true to size. I think I ordered 2XL with my hernia, but my waist is about 40”. They do loosen up a bit the longer you wear them. Washing/drying them tightens them back up. I bought 2 and change them out every 5-7 days or maybe sooner if I sweat a lot in them.
