Seat belt placement for stomas?

This topic is about finding ways to position a seat belt comfortably and safely for people with a stoma while driving. The challenge is that the lap section of a standard seat belt can press on the stoma, causing discomfort and potentially restricting the flow of the pouch. Here are some suggestions and insights shared by others who have faced similar issues:

1. General Safety
- It's important for the lap belt to sit low and snug across the pelvis. Wearing it across the stomach or knee can lead to severe injury in a crash.
- Some people carry a doctor's letter or medical certificate to exempt or modify seat-belt use, though success with law enforcement varies.

2. Simple, Inexpensive Modifications
- Use a clip or clothes-peg on the shoulder-belt pivot to prevent the belt from tightening.
- Seat-belt positioning clips, often used by children or pregnant women, can move the lap belt below the stoma.
- Placing foam or fabric pads, rolled towels, or small items like ice-cream tubs between the belt and abdomen can reduce pressure.
- Seat-belt extenders can relocate the lower anchor point, easing pressure on the abdomen.

3. Purpose-Built Stoma Protection
- Devices like "Stoma Guard" or "Stoma Shield" can be worn on an ostomy belt.
- The Stoma-Dome is a hard plastic dome that attaches to the pouch, allowing for drainage without removal.
- A hard-plastic guard from Ostomy Resolutions is reusable and costs around $50.
- Custom-made carbon-fiber guards are reported to be strong and comfortable.

4. Vehicle or Belt Redesign Ideas
- Some new buses have belts that hang from above to avoid abdominal pressure.
- Custom extensions can be fabricated to arch over the stoma, acting as a built-in guard.

5. Alternative Ostomy Management While Driving
- Irrigating can eliminate most output and seat-belt concerns for those who can do it.
- A two-piece appliance with a tube-exit bag connected to a container allows continuous drainage without needing pouch access.

6. Practical Work-Arounds
- Fasten the belt before sitting, then slip in and use only the shoulder strap across the chest.
- Some route the shoulder strap normally and keep the lap belt behind them or under the buttocks, avoiding police issues if the chest strap is visible.

7. Other Products Mentioned
- Hovding inflatable cycling airbags were noted in a side discussion about personal safety devices.
