Bleeding Around Stoma with Long-term Ileostomy: Advice?

This topic is about someone who has had an ileostomy for six years and has recently noticed some bleeding around the base of their stoma. They are seeking advice on why this might be happening and what they can do about it.

Here are some helpful insights and advice:

1. Common mechanical causes:
- The hole in the wafer or baseplate might be cut too small or not aligned properly, which can cause the plastic edge to act like a "paper cut."
- Pressure on the wafer during activities like bending or lifting can press it against the stoma.
- Being too rough when wiping or cleaning the area can also cause bleeding.
- It's a good idea to check the current size of the stoma, recut a slightly larger opening, and ensure everything is positioned correctly. Using barrier rings or paste can provide extra cushioning.

2. Occasional minor bleeding is normal:
- The stoma lining is made of delicate intestinal tissue that has a rich blood supply.
- A light bleed can often be stopped with a puff of stoma powder.

3. When to seek professional help:
- If the bleeding is continuous or heavy, it's important to see a stoma or wound-care nurse promptly.
- If bleeding points persist, silver nitrate cauterization might be used, as some have experienced.

4. Other tips:
- Keep an eye on any recent changes in products or routines that might be irritating the stoma.
- If you're unsure of the cause or if the bleeding happens again, it's best to contact a stoma therapy nurse for an assessment and peace of mind.
