Need help with pancaking and leakage from colostomy!

The topic at hand is about dealing with pancaking and leakage issues for someone with a colostomy. Pancaking occurs when stool sticks to the stoma and doesn't drop into the pouch, which can lead to leaks. Here are some helpful tips and advice to manage this situation:

1. **Prevent Air Loss and Maintain Space in the Pouch**
- If your pouch has a filter, cover it with the provided sticker or some extra tape to slow down air escape.
- Before putting on a new pouch, blow a little air into it and try to keep some air inside while wearing it.

2. **Re-distribute Stool with Lubricant**
- Keep using the baby oil method. Occasionally tip the pouch upright, rub it between your hands to move the oil back to the top, and massage the stool towards the bottom when needed.
- You can also use commercial lubricant-deodorizer drops for the same purpose.

3. **Hydration and Stool Consistency**
- Drink plenty of water to help keep the output softer and less likely to stick.
- Consider using fiber supplements or products like Miralax to help bulk or soften the stool. Some people find fiber pills helpful.

4. **Mechanical Pouch Care**
- Each time you empty the pouch, pour some water (about one-third of a 20-ounce bottle) into it, swish it around, and drain to remove residue and reduce pancaking.

5. **Equipment Changes**
- Convex pouches can be beneficial if your stoma tends to retract.
- Adding a Convatec Stomahesive Seal under the wafer can create a more secure and leak-resistant fit. Warm it in your hand or under your arm before molding.

6. **Irrigation Option**
- For those with suitable sigmoid or end colostomies, regular colostomy irrigation can almost eliminate pouch output for 24 hours, preventing pancaking. Discuss this option with a Wound, Ostomy, and Continence Nurse (WOCN).
